Job summary

CAMBA is seeking a Case Manager (HRA) in Brooklyn, NY to review tenant eligibility, create and maintain tenant files, and conduct initial intake and ongoing assessments. You will collaborate with tenants to develop independent living plans, locate community resources, and advocate for access to needed services to help tenants achieve stable housing and self-sufficiency.

The role requires a Bachelor's degree with experience serving SPMI populations and the ability to obtain fingerprint clearance.

Qualifications

• Bachelor's degree required with experience working with seriously and persistently mentally ill population (SPMI).
• Obtain required background checks (CHC fingerprinting) and clearance.
• Commitment to mandated reporting and client advocacy as part of case management responsibilities.

Responsibilities

• Review documentation establishing tenants' eligibility and maintain files.
• Conduct initial intake and assessments; develop and revise independent living plans with tenants.
• Identify community resources and make referrals to services; support tenants toward self-sufficiency.
• Monitor progress through regular contact and visits; document outcomes.
• Participate in case conferences and prepare reports for oversight and program evaluation.
• May schedule appointments and accompany tenants to referrals and appointments.
• May assist with benefits applications and data entry into databases.
