Job summary

CAMBA is seeking a Case Manager in Brooklyn, NY to form plans for permanent housing placement and to engage clients needing social service assistance. You will interview and assess clients, create Independent Living Plans, and refer to community resources.

The role requires a Bachelor’s degree (or equivalent), potential First Aid/CPR and overdose prevention certifications, and a preference for bilingual English/Spanish. Full-time, with CAMBA benefits included.

Qualifications

• Bachelor's degree (e. g., B.A., B.S.W.) and/or equivalent experience.
• May be required to become First Aid/CPR certified.
• May be required to become certified in overdose prevention.
• Bi-lingual preferred.
• Example-Bi-lingual English and Spanish.

Responsibilities

• Immediately formulate a plan and prepare clients for permanent housing placement.
• Engage individuals requiring social service assistance.
• Interview and evaluate clients; formulate Independent Living Plans and goals.
• Locate and refer to appropriate community resources for clients.
• Maintain professional relationships with clients and client confidentiality.
• Provide support, training and coordination with other social services staff.
• Assist clients in advocating for themselves to gain housing stability.
• Monitor clients' progress toward their goals via face-to-face contacts/sessions.
• Document all client encounters via progress notes and update CARES Database.
