Job summary

CAMBA in Brooklyn is seeking a dedicated Case Manager to support formerly homeless families and individuals through intake, assessment, planning, and referrals. You will coordinate with tenants, partner agencies, and CAMBA programs to promote self-sufficiency and access to housing, health, and employment resources.

This role requires strong confidential documentation, ability to navigate social services, and commitment to equity.

Qualifications

• Experience working with the Seriously and Persistently Mentally Ill (SPMI) population or equivalent.

Responsibilities

• Maintain professional relationships with clients and ensure confidentiality.
• Review tenant eligibility documentation and maintain files.
• Conduct intake and assess tenants' needs; develop living plans.
• Identify community resources and refer tenants to services.
• Document progress notes and participate in case conferences.
• Follow up with tenants and referral organizations to support progress.
