Job summary

CAMBA in Brooklyn, NY is hiring a Housing Case Manager for our Short Term Housing Program. You will conduct prescreening to determine eligibility, gather verification documents, and complete intakes, assessments and service plans to support clients transitioning to stable housing.

You will document progress, coordinate with medical and social services, escort clients to appointments, and assist with benefits applications.

Qualifications

• Bachelors Degree OR Associates degree/high school diploma/GED with at least 4 years’ experience in case management or related social service field.

Responsibilities

• Prescreen clients to determine program eligibility and gather required documentation.
• Complete intakes, assessments, service plans, reassessments, and updates.
• Enter client services into eSHARE and maintain audit-ready charts.
• Connect clients to medical, mental health, housing and legal services.
