Job summary

Qualifications

• Minimum: Bachelor’s degree with one year of experience or equivalent.
• Excellent written and verbal communication skills.
• Computer literacy in Microsoft Office Suite; ability to document clearly in records.

Responsibilities

• Conduct intake assessments, develop Service Plans, perform reassessments and updates.
• Enter client services into Client Track and maintain accurate records.
• Connect clients to medical, mental health, housing, legal and other entitlements.
• Monitor progress toward goals via calls and home/office visits; document progress.
• Escort clients to appointments and coordinate with outside organizations.
