Job summary

Zumbro Valley Health Center Inc is seeking a Full-time Housing Case Manager to provide Housing Support and Case Management to individuals experiencing homelessness. The role demands compassion, organization, and teamwork to empower clients toward stability and independence.

Qualifications

• Bachelor's degree in Social Work, Psychology or related human services field.
• 1 year of experience providing services to individuals with a mental illness or substance use disorder required.
• Must qualify as a Mental Health Practitioner in the State of MN.
• Licensed as a Social Worker in the State of MN preferred.
• Provisional License in the State of MN will be considered.

We are looking for a Full-time Housing Case Manager to provide Housing Support and Case Management services to individuals that have a history or are currently experiencing homelessness.
