Zumbro Valley Health Center Inc. in Rochester, MN seeks a full-time Mental Health Practitioner to work the day shift, providing compassionate care to adults with severe mental illness and co-occurring disorders.

You will work with a multidisciplinary team to monitor symptoms, support independent living, and ensure medication compliance. Responsibilities include admissions assessments, treatment planning, crisis response, and documentation of shift notes, paperwork, and referrals.

Qualifications

• Bachelor’s degree in psychology, social work or related field.
• 1 year (2000 hours) experience with adults with severe mental illness/chemical dependency.
• Must qualify as Mental Health Practitioner in MN; valid driver's license and auto insurance.

Responsibilities

• Assess strengths and needs of clients upon admission.
• Assist with independent living skills including shopping, meals, and housekeeping.
• Monitor and document physical and emotional status; medication compliance.
• Facilitate residential program activities, classes and groups.
• Complete treatment plans, shift notes, admission/discharge paperwork, and financial forms.
• Respond to crises using verbal and non-verbal techniques.

As a Mental Health Practitioner, you will provide care and support to clients with severe mental illness and co-occurring disorder.You will work alongside a team of professionals to provide services in the areas of mental health, co-occurring disorders, monitoring medication compliance, and independent living skills.
