Zumbro Valley Health Center Inc is seeking a Full-time Housing Case Manager to provide Housing Support and Case Management to individuals experiencing homelessness. The role demands compassion, organization, and teamwork to empower clients toward stability and independence.

Responsibilities include coordinating services, referrals, and follow-up; completing admission and discharge paperwork; participating in case consultations; assisting clients with securing housing, interfacing with landlords,

Qualifications

• Bachelor's degree in Social Work, Psychology or related human services field.
• 1 year of experience providing services to individuals with a mental illness or substance use disorder required.
• Must qualify as a Mental Health Practitioner in the State of MN.
• Licensed as a Social Worker in the State of MN preferred.
• Provisional License in the State of MN will be considered.

Responsibilities

• Coordinate referrals and follow up for services and resources as needed.
• Complete all admission and discharge paperwork in a timely manner.
• Attend and participate in case consultations, and supervision meetings.
• Assist clients with securing housing, and interface with landlords to maintain housing.
• Provide support services once housed to help improve stability and longevity in housing.

Education
