Job summary

Zumbro Valley Health Center in Minnesota is seeking a Full-time Housing Support Practitioner to provide supportive services and skills training within our housing program. You will screen potential tenants and collaborate with an integrated treatment team.

You will facilitate independent living skills in individual and group settings, advocate for clients, and ensure timely documentation of housing and skills sessions.

Qualifications

• 1 year of experience providing services to individuals with a mental illness or substance use disorder.
• Qualified as a Mental Health Practitioner in the State of MN.
• Licensed Social Worker in MN preferred; provisional license considered.
• Valid driver's license.

Job description

We are looking for a Full-time Housing Support Practitioner to provide supportive services and skills training to clients who are enrolled in a ZVHC housing program. The Housing Support Practitioner position will screen potential tenants for the program, work with an integrated team to identify clients who may benefit from additional CCBHC services and serve as a member of the Treatment Team.ResponsibilitiesBuild and maintain professional relationships with landlords.Screen potential clients to ensure they meet the qualifications for the program.Assist in identifying resources related to basic needs for housing maintenance.Advocacy and linkage to community resources.Provides independent living skills both individually and in group settings.Help develop and work will all members of the treatment plan team as often as needed.Facilitate skills training to clients individually and in a group setting.Coach, teach, educate, and model independent living skills to clients.Seek treatment consultation from Clinical Supervisor.Accurately completes all required documentation and notes for housing and skills sessions for each client in a timely manner.Requirements & QualificationsBachelor's degree in Social Work, Psychology or a related human services field.1 year of experience providing services to individuals with a mental illness or substance use disorder required.Experience working with housing clients preferred.Must qualify as a Mental Health Practitioner in the State of MN.Licensed as a Social Worker in the State of MN preferred.Provisional License in the State of MN will be considered.Valid driver's license and proof of auto insurance.Why Zumbro Valley Health Center?Since 1966, Zumbro Valley Health Center has been serving our community as a non-profit organization.
