Job summary

Zumbro Valley Health Center is seeking a Mental Health Practitioner to provide direct mental health services to adults in the community, at home, or at the office. You will collaborate with a dedicated team to ensure high-quality care.

Responsibilities include maintaining a caseload, developing treatment plans with the team, and facilitating individual and group sessions. Experience with severe emotional disturbances is a plus; valid MN license is required for eligibility.

Our team is seeking a Mental Health Practitioner to provide direct mental health services to adults out in the community, at their home, or at the office. You will work collaboratively with a dedicated team of professionals to provide the highest quality of care possible.

Responsibilities

• Maintain a caseload of clients.
• Help develop and work will all members of the treatment plan team as often as needed.
• Facilitate services to clients individually and in a group setting.
• Coach, teach, educate, and model independent living skills to clients.
• Coordinate and make further referrals for clients as needed.
• Seek treatment consultation from Clinical Supervisor.
• Accurately completes all required documentation and notes for each client in a timely manner.

Qualifications

Requirements & Qualifications

• Bachelor’s degree in Social Work, Psychology or a related human services field.
• Minimum of two years providing services to individuals with a mental illness or substance use disorder required.
• Must qualify as a Mental Health Practitioner in the State of MN
• Experience working with severe emotional disturbances in adults is preferred.
• Valid driver's license and proof of auto insurance.

Pay: $25.11 - 30.13 per hour. Compensation is based on years of experience.
