Job summary

Zumbro Valley Health Center seeks a Mental Health Practitioner to deliver direct mental health services to adults in the community, at home, or at the office. You will work with a dedicated team to provide high-quality care and support diverse adult clients.

The role involves maintaining caseloads, developing treatment plans with colleagues, and facilitating individual and group sessions. A MN qualification and a valid driver's license are required to serve this region.

Qualifications

• Bachelor's degree in Social Work, Psychology or related field.
• Minimum of two years providing services to individuals with a mental illness or substance use disorder.
• MN Mental Health Practitioner qualification required.
• Valid driver's license and proof of auto insurance.

Responsibilities

• Maintain a caseload of clients.

You will work collaboratively with a dedicated team of professionals to provide the highest quality of care possible.ResponsibilitiesMaintain a caseload of clients.Help develop and work will all members of the treatment plan team as often as needed.Facilitate services to clients individually and in a group setting.Coach, teach, educate, and model independent living skills to clients.Coordinate and make further referrals for clients as needed.Seek treatment consultation from Clinical Supervisor.Accurately completes all required documentation and notes for each client in a timely manner.Requirements & QualificationsBachelor’s degree in Social Work, Psychology or a related human services field.Minimum of two years providing services to individuals with a mental illness or substance use disorder required.Must qualify as a Mental Health Practitioner in the State of MNExperience working with severe emotional disturbances in adults is preferred.Valid driver's license and proof of auto insurance.Why Zumbro Valley Health Center?Since 1966, Zumbro Valley Health Center has been serving our community as a non-profit organization.
