Qualifications

• Bachelor's degree in Social Work, Psychology or a related human services field.
• 1 year of experience providing services to individuals with a mental illness or substance use disorder.
• Mental Health Practitioner licensure in MN preferred; Social Worker licensure in MN preferred; provisional licensure considered.

Responsibilities

• Coordinate, make referrals, and follow up for services and resources as needed.
• Complete admission and discharge paperwork in a timely manner.
• Attend and participate in case consultations and supervision meetings.
• Assist clients with securing housing and interface with landlords to maintain housing.
• Provide support services after housing is secured to improve stability and longevity.
• Use motivational interviewing to build rapport with clients.
