Job summary

The People Concern is seeking a dedicated Case Manager to assist Santa Monica households at risk of homelessness. The role focuses on flexible financial assistance and case management to prevent housing loss and connect participants with essential services.

You will perform outreach, assess barriers, collect documentation, and coordinate with legal and community partners to stabilize housing and support clients toward independence.

Qualifications

• High School Diploma or GED is required.
• 1 year of case management or customer service experience preferred.
• Ability to communicate with subcontractors, service providers and community partners in a culturally competent manner.
• Valid CA Driver's License and access to reliable personal vehicle.

Responsibilities

• Initiate outreach efforts within the community to reach high-risk participants.
• Educate participants about local resources and assist with completing the FFA online application.
• Assess housing barriers for unstably housed households to determine needs.
• Complete prioritization tools to determine vulnerability and risk level for each participant.
• Collect documentation to verify eligibility and tailor level of assistance accordingly.
• Coordinate with Central Fiscal Agent and City Right to Counsel to submit financial and legal referrals timely.
• Develop individualized housing stabilization goals including linkages to employment, legal, mental health, medical, and other services.
• Advocate with landlords to develop workable plans to maintain housing.
• Maintain clear verbal and written communication among all parties involved.
• Work with legal service entities to connect case management with legal support.
• Link participants to credit counseling and other services for budgeting and improving credit.
• Document all services, referrals, and participant feedback.
• Advocate for and assist individuals in obtaining benefits, housing referrals, and other support.
• Provide reunification or relocation assistance when necessary.
• Perform other duties as assigned.
