This role emphasizes person-centered planning, timely documentation, and group activities, requiring strong communication and teamwork; bilingual Spanish is preferred.

Qualifications

• Minimum of a high school diploma or equivalent required; bachelor’s degree in related field preferred.
• Excellent oral and written communication skills.
• Ability to prioritize, meet deadlines, and work independently or in a team.

Responsibilities

• Provide engagement and comprehensive support to program participants in supported housing.
• Support participants using a person-centered model to achieve housing stability and personal goals.
• Plan and lead community building group activities; document assessments and service plans on time.
