Job summary

TURN in Escondido, CA seeks a Housing Specialist to provide case management and linkage to medical, educational, social, vocational, and housing services. You will coordinate referrals and advocate for clients, supporting whole-person care and service access.

The role requires a valid California driver’s license, 2 years’ experience or an AA in social services, and familiarity with Microsoft Office and EHR systems.

Qualifications

• AA degree in social service or related field or High School Diploma and 2 years’ experience.
• Valid CA driver’s license and current auto insurance per contract requirements.
• Experience providing mental health and/or substance use disorder services preferred.
• Knowledge of Microsoft Office and EHR software preferred.

Responsibilities

• Provide case management with crisis intervention, discharge plan, group & individual work, mental health/substance use disorder counseling, screening, and assist clients in meeting basic needs and symptom management.
• Complete accurate and timely documentation, meet minimum billable requirements for direct client services if contractually required.
• Facilitate placement including pre-placement visits, and transportation as needed.
• Shift coverage including part of the on-call team.
• Provide field-based services as required by program and client need.
• Maintain confidentiality of client’s care.
• Participate in scheduled program meetings.
• Other relevant duties as assigned.
• Build and maintain partnerships with community and housing agencies.
• Track and monitor HMIS, AppFolio, and other program systems.
