Job summary

Concern Housing is seeking a Behavioral Health Case Manager to connect residents with community resources, provide counseling and daily living skills training, and ensure thorough documentation and compliance with program requirements. This role emphasizes advocacy and client-centered support in a non-profit housing setting.

The position requires a Bachelor’s degree in a related mental health field and at least two years of experience.

Qualifications

• Bachelor’s degree preferred in Psychology, Rehabilitation, Social Work or related mental health discipline and at least 2 years’ experience in similar setting.
• Strong verbal and written communication skills; ability to make sound judgments regarding client care.

Responsibilities

• Facilitate residents linkage with services available in the community.
• Develop comprehensive needs assessments and support plans; maintain client records.
• Provide individual counseling and support services.
• Provide medication management services.
• Engage in recovery planning.
• Provide daily living skills training.
• Attend supervision and staff meetings.
• Maintain agency compliance documents and trainings.
• Other duties as assigned by the Program Supervisor.
