Job summary

Qualifications

• 5+ years of case management experience with vulnerable populations.
• Valid Driver's License.
• Use of personal vehicle.
• HS diploma or higher; degree preferred.
• Experience with case management databases, HMIS preferred.

Responsibilities

• Screen clients upon admission and complete intake packets.
• Meet clients to assess needs and create a case plan.
• Coordinate with agencies and community resources for referrals.
• Monitor progress, update plans, and ensure goals are met.
• Maintain confidentiality and accurate documentation per policies.
• Educate clients and families about available resources.
• Provide advocacy and crisis intervention when needed.
• Attend community meetings and care conferences.
• Visit agencies, shelters, and hospitals to assess housing needs.
