Job summary

Pacific Clinics in Pasadena, CA seeks a Housing Navigator to ensure timely and quality housing services for members authorized by Managed Care Plans. You will collaborate with CoCs, MCPs, and ECM providers to deliver housing supports, deposits, and tenancy services across the county.

The role requires knowledge of HUD funding and housing resources, and experience in housing advocacy. This is a field-based, on-site position with regular team collaboration.

Qualifications

• High School Diploma or GED with 4+ years of relevant experience.
• Knowledge of housing rights pertaining to homelessness, and HUD funding.
• Training in Motivational Interviewing, Trauma-Informed Care, Harm Reduction.
• Ability to work effectively with diverse communities and coordinate with multiple partners.

Responsibilities

• Develop outreach strategies for consumers who tend to isolate themselves.
• Support consumers with housing deposits and tenancy and sustaining services.
• Identify and secure subsidized housing resources and review housing options.
• Coordinate with HUD funding, CES, CoC homeless services, MCPs and ECM providers.
• Maintain documentation in HMIS and Salesforce and complete required HUD reports.
