You will conduct intake, develop ISP, coordinate services, maintain records, and ensure confidentiality, with driving required and a full-time on-site schedule.

• Provide referrals and support to resources to meet ISP goals.
• Complete program assessments (e.g., VI-SPDAT).
• Perform 90-day reviews and maintain accurate participant files.
• Maintain confidentiality per HIPAA and policy guidelines.
• Develop relationships with clients and partners to support housing goals.
