Job summary

St. John’s Community Health is seeking a Housing Care Manager to join the Community Supports Housing Program in Los Angeles.

The role focuses on Housing Navigation for Medi-Cal members, including assessments, support plans, and assistance with securing housing and necessary documentation. The position also delivers Tenancy Services to maintain stable tenancy, with emphasis on advocacy, budgeting, and coordination with landlords.

Qualifications

• Bachelor's degree or higher is required.
• Familiar with housing resources such as Veteran Services, Rapid Rehousing, and Section 8.
• Knowledge of housing navigation skills and lease compliance.
• 2 years of social services/public health experience.
• Experience serving underserved communities including homeless populations and people with mental illness.
• Valid CA Driver's License and vehicle for work-related travel.
• Bilingual in English and Spanish preferred.

Responsibilities

• Deliver Housing Navigation services to Medi-Cal members prior to tenancy, including assessments and individualized housing support plans.
• Provide tenancy services after housing is secured, including landlord relations, lease compliance, and eviction-prevention coaching.
