Advocate Health, based in Charlotte, NC, seeks a Part-time Social Worker to join Care Management. The role focuses on psychosocial assessments, supporting patients post-hospitalization, and coordinating care across the continuum, with collaboration from the Clinical Care Management team.

Requirements include a Master of Social Work (MSW) and preferred LCSW; at least 1 year of hospital experience and NASW ethics adherence. On-call duties may apply. Salary aligns with part-time schedule.

Qualifications

• Masters in Social Work is required; LCSW preferred.
• Applicable state Certification is preferred.
• At least 1 year professional hospital or health setting experience preferred.
• Adherence to National Association of Social Workers Code of Ethics is expected.

Responsabilités

• Completes psychosocial assessments for post-hospital care of designated patients and their support system.
• Assists patients in coping with stress related to hospitalization, disability, chronic/terminal illness.
• Works with the Clinical Care Management team to assess and evaluate the appropriate level of care across the continuum.
• Serves as a resource to hospital staff and physicians regarding emotional and social aspects of illness and social support system.
• Provides information and education to patients on community resources and post-hospital care options; makes referrals as needed.
• Reports suspected child/adult abuse, neglect, or exploitation; liaises with Department of Social Services during evaluation.
• Responds to requests for consultative services to patients after normal working hours.
