Job summary

Advocate Health seeks a skilled Social Worker to perform psychosocial assessments and support planning for post-hospital care. You will collaborate with care management and hospital staff to ensure appropriate levels of care and access to community resources.

The role emphasizes patient advocacy, stress coping support, and after-hours consults to address complex social and emotional health needs in a hospital setting.

Qualifications

• Masters in Social Work required; LCSW preferred.
• Adherence to NASW Code of Ethics.
• At least 1 year professional hospital or health setting experience preferred.

Responsibilities

• Completes psychosocial assessments for post hospital care for designated patients and their support system.
• Assists patients in coping with stress related to hospitalization, disability, chronic/terminal illness.
• Works with Clinical Care Management to determine appropriate level of care or environment setting.
• Serves as a resource to hospital staff and physicians regarding emotional, social, and psychosocial aspects of illness and its effects on social support systems.
• Provides information and education to patients on community resources and post-hospital care options; makes referrals to community agencies as needed.
• Reports suspected cases of child and adult abuse/neglect/exploitation; liaises with hospital and Department of Social Services during evaluation/investigation.
• Responds to requests for consultative services to patients after normal working hours.
