Ideal candidates hold an MSW (LCSW preferred), with hospital experience and strong ethical standards. This role involves collaboration, outreach, and after-hours consults when needed.

Qualifications

• Master of Social Work required; LCSW preferred.
• Minimum 1 year hospital or health setting experience preferred.
• Adherence to NASW Code of Ethics.

Responsibilities

• Complete psychosocial assessments for post-hospital care.
• Assist patients in coping with stress from hospitalization.
• Collaborate with Clinical Care Management to determine level of care.
• Provide information and referrals to community resources; educate patients.
