MSW is required; LCSW preferred; at least 1 year hospital experience preferred. PRN days and some weekend coverage; pay range $30.70–$46.05 per hour; benefits eligibility varies.

Qualifications

• Master’s degree in Social Work (MSW) required.
• LCSW licensure preferred.
• At least 1 year of professional hospital or healthcare experience preferred.

Responsibilities

• Complete psychosocial assessments for post-hospital care for designated patients and their support systems.
• Assist patients in coping with stress related to hospitalization, disability or chronic illness.
• Collaborate with Clinical Care Management to determine appropriate level of care and environment settings.
• Serve as a resource to staff and physicians on emotional and social aspects of illness and impact on support networks.
• Provide information and education on community resources and post-hospital care options.
• Make referrals to community agencies as needed.
• Report suspected child and adult abuse/neglect/exploitation.
• Liaise with hospital, patients and Department of Social Services during evaluation/investigation.
• Respond to after-hours consultative requests.
