Job summary

Advocate Health is seeking a qualified Social Worker (MSW/LCSW preferred) to provide education and support to patients, families, and staff. You will assess needs, plan and coordinate post-hospital care, and evaluate services to empower individuals and their families.

Responsibilities include completing psychosocial assessments, assisting with coping strategies, collaborating with the Clinical Care Management team, and serving as a resource for staff on emotional and social aspects of illness.

Qualifications

• Must have MSW; LCSW preferred and current licensure in state.
• NC Certification preferred.
• Minimum 1 year of professional hospital or health setting experience; familiarity with ethics in social work.

Responsibilities

• Performs psychosocial assessments for post-hospital care for designated patients and their support system.
• Assists patients in coping with stress related to hospitalization, disability, or chronic/terminal illness.
• Collaborates with Clinical Care Management to determine appropriate level of care across the continuum.
• Serves as a resource to hospital staff and physicians on emotional, social, and psychosocial aspects of illness and its effects on the patient’s support system.
• Provides information and education to patients on community resources and post-hospital care options; makes referrals as needed.
• Reports suspected cases of abuse/neglect/exploitation and acts as liaison with Department of Social Services during evaluation.
• Responds to requests for consultative services outside regular hours.
