Job summary

Advocate Health in Charlotte, NC is seeking a Social Worker to conduct psychosocial assessments and support patients transitioning from hospital to home or other care settings.

The role requires a Master’s in Social Work, with LCSW preferred. You will work with the Clinical Care Management team and hospital staff to ensure appropriate care levels and resource connections, including community referrals and family support.

Qualifications

• Masters in Social Work required.
• LCSW preferred.
• Experience in hospital or health setting preferred.
• Expertise with data management tools.

Responsibilities

• Complete psychosocial assessments for post-hospital care.
• Assist patients in coping with stress related to hospitalization or disability.
• Collaborate with the Clinical Care Management team to assess appropriate level of care.
• Serve as a resource to hospital staff on emotional, social, and psychosocial aspects of illness.
