The role requires a Master’s in Social Work, with LCSW preferred, and at least two years of healthcare social work experience. This is a full-time, onsite position with weekend coverage as needed.

Qualifications

• MSW degree from CSWE-accredited program.
• Active social work license or eligibility; LCSW preferred.
• Minimum two years’ social work experience in healthcare.

Responsibilities

• Conducts comprehensive psychosocial assessments and discharge planning.
• Provides social work services, including evaluation of needs and follow-up.
• Collaborates with care teams to coordinate post-acute placement and resources.
• Communicates with payers and CMS-related bodies as needed to support care coordination.
• Maintains regulatory compliance and professional ethics in all duties.
