Job summary

Advocate Health seeks a Social Worker with an MSW to support patients and families after hospitalization. You will perform psychosocial assessments, connect patients with resources, and advocate for appropriate care settings.

In collaboration with the Clinical Care Management team, you will educate patients about options, report abuse as needed, and provide after-hours consults. Hospital experience and NASW ethics knowledge preferred.

Qualifications

• Masters in Social Work required; LCSW preferred.
• Experience in hospital or health-related setting preferred.
• Familiarity with NASW Code of Ethics recommended.
• Ability to educate and connect patients with community resources.

Responsibilities

• Completes psychosocial assessments for post-hospital care for designated patients and their families.
• Assists patients in coping with stress related to hospitalization, disability, chronic/terminal illness.
• Collaborates with Clinical Care Management to determine the appropriate level of care across the continuum.
• Serves as a resource to hospital staff on emotional and social aspects of illness and social support.
• Educates patients on community resources and makes referrals as needed.
• Reports suspected abuse/neglect/exploitation and liaises with Department of Social Services.
• Responds to after-hours consult requests for patient care.
