Job summary

Advocate Health, operating as Atrium Health in the Carolinas, seeks a part-time social worker to support post-hospital care planning and family counseling. You will join the Care Management team to evaluate patient needs and arrange appropriate care settings, education, and referrals.

On-site role with weekend coverage and on-call duty as scheduled. The candidate brings a MSW with current licensure and at least 1 year in hospital or health settings, plus NASW ethics adherence, to support

Qualifications

• Masters in Social Work required; LCSW preferred.
• At least 1 year professional hospital/health setting experience.
• Adherence to NASW Code of Ethics.
• State social work certification preferred.

Responsibilities

• Completes psychosocial assessments for post-hospital care for designated patients and their families.
• Assists patients in coping with stress related to hospitalization, disability, or chronic illness.
• Collaborates with Clinical Care Management to determine appropriate level of care across the continuum.
• Serves as a resource to staff and physicians regarding social and emotional aspects of illness and support needs.
• Educates patients on community resources and post-hospital care options; makes referrals as needed.
• Reports suspected abuse/neglect/exploitation; liaises with DFS during evaluation/investigation.
• Responds to consult requests after normal working hours.
