Job summary

Covenant Care, Corp. in Jacksonville, FL is seeking a Concierge Home Care professional in a full-time hybrid role to strengthen hospital-to-home transitions and grow referrals.

You will build relationships with hospital teams, educate families about home health services, and coordinate care through discharge.

Qualifications

Responsibilities

• Build hospital and referral relationships with executive and wellness directors.
• Educate referral sources on home health services and eligibility.
• Support referral growth and admissions goals.
• Meet patients at bedside prior to discharges.
• Explain home health services, benefits, and insurance coverage.
• Coordinate care from hospital to home with PCPs, caregivers and DME.
• Ensure timely admissions documentation and referral activity tracking.
• Advocate for safe, seamless discharges and reduce readmissions.

Qualifications
