Job summary

Concierge Home Care in Melbourne, FL is seeking a Clinical Care Coordinator, RN to support the clinical team and oversee day-to-day operations in a primarily office-based, Monday–Friday role.

You will guide clinicians, ensure Medicare compliance, and drive quality and patient-centred care while coordinating with physicians and support staff. A Florida RN license and at least 1 year home health experience are required.

Qualifications

• Active, unencumbered Florida RN license.
• Minimum 1 year of RN experience.
• At least 1 year of skilled home health experience.
• Strong understanding of home health regulations, clinical documentation, and Medicare compliance.
• Strong clinical judgment and communication skills.
• Ability to coach, support, and collaborate with clinical professionals.

Responsibilities

• Support and coordinate the day-to-day clinical operations of a home health team
• Provide guidance, support, and coaching to home health clinicians
• Review clinical documentation for Medicare and regulatory compliance
• Monitor quality of care, patient outcomes, and clinical performance
• Collaborate with physicians and interdisciplinary team members

Concierge Home Care is looking for a Clinical Care Coordinator, RN to support the clinical team.

This is an opportunity to use your clinical expertise in a Monday-Friday, primarily office-based role while helping clinicians succeed and ensuring patients receive safe, compliant, high-quality home health services.

What You'll Do

• Competitive salary: $80K-$85K (based on experience)
• Quarterly performance bonus opportunities

Schedule

• Monday-Friday
• Primarily office-based
• Local travel as needed to support clinical operations
• Leadership and clinical coordination responsibilities without a traditional bedside schedule
• Three weeks PTO, increasing to four weeks after five years
• PTO accrual begins the first day of employment
