Hospital & Referral Relationships

Build relationships with:

• Case managers
• Hospital staff

Educate referral sources on:

Support referral growth and admissions goals

Patient & Family Support

Meet patients bedside prior to discharge

Explain home health services and benefits

Educate on:

• Insurance coverage
• Skilled home care services

Transitional Care Coordination

Coordinate care from hospital to home

Collaborate with:

• DME providers
• PCPs
• Caregivers

Partner closely with:

• Intake teams
• Clinical managers
• Schedulers
• Account Executives

Provide timely referral and patient updates

Ensure continuity of care throughout the admission process

Referral & Documentation Management

Process referrals in real time

Complete admission documentation accurately and in a timely manner

Track:

• Referral activity
• Conversion rates

Qualifications

Required

• Active Florida RN or LPN license
• Strong communication and interpersonal skills
• Detail-oriented and organized
• Valid driver’s license and reliable transportation

• Home health experience
• Hospital liaison or healthcare marketing experience
• Case management or transitional care experience
• Knowledge of payer sources and referral processes
