Responsibilities

• Interprets medication orders to ensure safety and proper use of medications and coordinates solutions with pharmacy, nursing and prescribers.
• Monitors therapy for drug interactions and therapeutic duplications; intervenes as needed.
• Evaluates medications for appropriate outcomes and communicates potential problems to prescribers.
