Job summary

Northwestern Medicine seeks a Clinical Pharmacist to ensure safe, effective medication use across patient care. You will interpret orders, monitor for drug interactions, adjust therapy with clinical judgment, and educate patients and staff.

Qualifications

• Bachelor’s Degree in Pharmacy or a Doctor of Pharmacy Degree required.
• Active Clinical Pharmacist license in the state of Illinois (IDFPR) preferred or required.

Responsibilities

• Interprets medication orders to ensure safety and proper use of medications; coordinates solutions with pharmacy, nursing, and prescribers.
• Monitors medication therapy for significant drug–drug, drug–diet, or drug–laboratory interactions and intervenes as needed.
• Evaluates and documents that selected medications achieve the desired therapeutic outcome and communicates issues to the prescriber.
• Acts as a consultant in pharmacokinetics and adverse reactions; participates in therapeutic drug monitoring and adjusts therapy as indicated.
• Processes medication order information into the Electronic Health Record to ensure accuracy of records.
