Job summary

Northwestern Medicine seeks a Clinical Pharmacist PNE to integrate medication therapy management within the hospital. The role emphasizes safety, efficacy, and collaboration with physicians, nurses, and other staff to optimize patient care.

Responsibilities include drug monitoring, pharmacokinetics consultation, and ensuring accurate documentation in the EHR. A state license and hospital experience are required to deliver high-quality pharmacotherapy services.

Qualifications

• Active Clinical Pharmacist license in Illinois (IDFPR).
• Bachelor’s Degree in Pharmacy or Doctor of Pharmacy degree.
• Experience/training in specialty areas (neonatology, critical care, oncology) preferred.

Responsibilities

• Interprets medication orders to ensure safety and proper use; coordinates problem solving between pharmacy, nursing, and prescribers.
• Monitors therapy for significant drug interactions; intervenes when necessary.
• Evaluates medication choices for therapeutic outcomes; communicates issues to prescribers.
• Acts as consultant on pharmacokinetics and adverse reactions to healthcare professionals.
• Processes medication orders into the EHR to ensure accurate records and charging.
• Maintains comprehensive patient profiles and oversees dispensing operations.
