• Interprets medication orders to ensure safety and proper use of medications and resolves related problems.
• Monitors medication therapy for significant interactions and intervenes as needed.
• Evaluates and documents appropriateness of medications for therapeutic outcomes.
• Acts as a consultant on pharmacokinetics and adverse reactions.
• Processes medication orders into the Electronic Health Record to ensure accurate records.
