Job summary

Memorial Hospital West in Florida is seeking a skilled Case Manager/Social Worker to develop individualized care plans from admission through discharge, coordinating with physicians, nurses and the multi-disciplinary team to ensure patients progress toward outcomes.

The ideal candidate has 1 year of hospital case management or social work experience and can participate in a Monday–Friday orientation period of 6 to 8 weeks.

Qualifications

• Masters: Social Work is required.
• BLS American Heart Association certifications (ECARD/RQI) are required/preferred.
• Florida LCSW licensure preferred; JDCH-specific licensure areas may apply.

Responsibilities

• Organizes and facilitates care conferences with patients, families, representatives, and other care team members.
• Works with patients/families to navigate post-acute options and insurance plans to coordinate care.
• Participates in discharge planning including coordinating patient transfers to other facilities and community resources.
• Collaborates with governmental agencies to ensure safe post-acute care.
• Collaborates with the interdisciplinary team and nursing staff to evaluate patient needs; initiates crisis interventions if needed.
