Job summary

Memorial Healthcare System is recruiting a Case Manager/Social Work professional to partner with physicians, nurses and the care team from admission to discharge. You will monitor progress toward outcomes and coordinate post-acute planning while addressing patient and family needs. No nursing position is implied.

The ideal candidate has 1 year of hospital Case Manager/Social Work experience and can work Monday–Friday, 8am–5pm, for 6–8 weeks of orientation.

Qualifications

• Master's degree in Social Work is required.
• BLS American Heart E-Card and related BLS credentials required.
• Florida Licensed Clinical Social Worker preferred.

Responsibilities

• Organizes care conferences with patients and families and care team.
• Works with patients to navigate the healthcare system and post-acute options.
• Participates in discharge planning and coordinates transfers.
• Collaborates with governmental agencies to ensure safe post-acute care.
• Collaborates with interdisciplinary team and nursing to reassess needs.
• Assesses and initiates crisis interventions and Baker Act if needed.
