UF Health is seeking a Case Manager to join our treatment team. The CM will participate in developing the patient's plan of care, facilitate throughput, and perform discharge planning and utilization review.

Qualifications

• RN licensure in Florida required for ASN/BSN path; MSW accepted in lieu.
• 1-2 years of discharge planning, case management, managed care, or RN experience in a medical setting.
• Education: ASN or MSW; BSN preferred.
• Strong verbal and written communication, organizational, and computer skills (20 wpm typing).

Responsibilities

• Participate in developing the patient's plan of care and discharge plans.
• Facilitate throughput and conduct utilization review.
• Promote appropriate utilization of resources and coordinate care with the team.
• Identify patient needs and establish safe discharge plans with caregivers.
