Job summary

Memorial Healthcare System seeks an experienced Case Manager/Social Worker to collaborate with physicians, nurses, and the multidisciplinary team from admission through discharge. The role focuses on developing individualized plans of care, monitoring progress, and addressing patient/family needs to ensure outcomes.

This position requires one year of hospital case management/social work experience and a 6–8 week orientation period.

Qualifications

• Masters in Social Work required.
• BLS American Heart E-Card required, BLS RQI ECARD preferred.
• BLS certificates/credentials listed above are required or preferred per job.
• Licensed Clinical Social Worker in Florida preferred.

Responsibilities

• Organizes care conferences with patients, families and care team.
• Navigates post-acute options and insurance with patients/families.
• Participates in discharge planning including transfers to other facilities.
• Collaborates with governmental agencies (DCF, APS, CMS, KePro) for post-acute care.
• Works with interdisciplinary team to evaluate patient needs regularly.
• Initiates crisis interventions when necessary (Baker Act).
