Qualifications

• Education: MSW required. LCSW preferred. Accredited Case Manager (ACM) preferred.
• Experience: Two years progressive experience in a department. Five years acute hospital social work experience.
• License: LCSW. Certification: BLS (AMERICAN HEART ASSN).
• Training: Must complete Tenet Case Management documentation system within 30 days of hire.

Responsibilities

• Facilitate care along a continuum through resource coordination to help patients access care and balance resources with self-determination.
• Assess patients for transition needs, including identifying those at risk for readmission.
• Conduct complex psycho-social assessments and interventions to promote timely throughput and safe discharge.
