Job summary

GBMC HealthCare is seeking a Triage RN to triage patient and caregiver calls, provide palliation and education, and coordinate with physicians, case managers, and pharmacies. The role supports terminally ill patients and families in collaboration with the care team.

The ideal candidate has a Diploma/ADN/BSN, Maryland RN licensure, and 1–2 years of nursing experience, preferably in home health hospice or triage settings. Strong communication and EMR skills are essential.

Qualifications

• Diploma/ADN/BSN from an accredited institution.
• Current Maryland State license as a Registered Nurse.
• One to two years of nursing experience; Home Health Hospice or triage experience recommended.

Responsibilities

• Triages all patient and caregiver calls, assisting in solutions and education.
• Serves as liaison with physician offices on behalf of the care team.
• Provides immediate response and emotional support to families at death calls.
• Assists with coordinating changes in patient level of care and communicates with insurance case managers.
• Documents discussions in detail in the EMR and coordinates equipment and pharmacy needs.
