Job summary

GBMC HealthCare seeks a Hospice Home Care Nurse in Towson, MD to assess patient and family needs and deliver nursing and supportive care for terminally ill patients. The role requires Maryland RN licensure, a diploma/ADN or BSN, and at least two years of experience.

Proficiency with EMR and strong communication are essential for coordinating care with the interdisciplinary team. Key duties include creating care plans, supervising home care delivery, and teaching families patient care skills

Qualifications

• Current Maryland State license as a Registered Nurse.
• Diploma, ADN and/or BSN from an accredited institution.
• Two years of nursing experience or equivalent combination of education and experience.
• Knowledge of infection control and safe nursing practices.
• Ability to manage time effectively and communicate with patients and families.

Responsibilities

• Completes initial assessment of patient and family to determine home care needs including physical, emotional, spiritual and mental status.
• Prepares and implements the care plan for patient/family unit with the Interdisciplinary Group.
• Provides direct nursing care to patients in conjunction with the primary physician.
• Plans and supervises delivery of care by the home health aide (HHA).
• Collaborates with Clinical Manager and updates physician and interdisciplinary team regularly.
• Uses problem oriented charting to ensure continuity of care in home or facility settings.
• Teaches family and facility staff nursing skills including pain control, injections, IV management, hygiene and vital signs.
• Ensures equipment arrangements for facility settings are made available.
• Attends to patient, family and facility at time of patient’s death.
