Job summary

GBMC HealthCare in Towson, MD is seeking a Triage RN who triages patient and caregiver calls, assists with symptom palliation, and provides education to families. You will coordinate with care teams, physicians, and insurance to ensure timely support and comfort for terminally ill patients and their loved ones.

Requires Diploma/ADN/BSN, Maryland RN license, and 1–2 years in Home Health Hospice or triage. Strong time management, communication, EMR skills, and the ability to work under pressure

Qualifications

• Diploma/ADN/BSN from an accredited institution.
• Maryland State RN license current.
• 1–2 years nursing experience; Home Health Hospice or triage experience preferred.

Responsibilities

• Triage all patient and caregiver calls and provide solutions and education.
• Serve as liaison with physician offices for the care team.
• Provide immediate emotional support to families at death calls.
• Assist with changes in patient level of care and coordinate with insurance case managers.
• Document discussions in detail in the EMR and coordinate with pharmacy on refills and scripts.
