Qualifications

Responsibilities

• Triage patient and caregiver calls, provide education and palliate symptoms.
• Serve as liaison with physician offices for the care team.
• Provide immediate emotional support to the family at time of death call.
• Coordinate changes in patient level of care as needed.
• Communicate with insurance case managers; document discussions in detail.
