Job summary

GBMC HealthCare is hiring a Triage Registered Nurse for Towson, MD. The RN triages patient and caregiver calls, provides symptom palliation and education, and coordinates with case managers, physicians, pharmacies, and DMEs to support terminally ill patients and families.

Requirements include Maryland RN licensure and 1–2 years of nursing experience, with hospice/triage background preferred. The role emphasizes patient- and family-centered care and collaboration within the care team.

Qualifications

• Diploma, ADN and/or BSN from an accredited institution.
• Current Maryland State license as a Registered Nurse.
• One to two years of nursing experience; Home Health Hospice or triage experience preferred.

Responsibilities

• Triages all patient and caregiver calls, assisting in solutions and education.
• Acts as liaison with physician offices on behalf of the care team.
• Provides emotional support to families at the time of death call.
• Assists with changes in patient level of care coordination.
• Communicates with insurance case managers and documents discussions in detail.
• Assists with pharmacy issues and orders supplies and durable medical equipment; documents in EMR.
• Coordinates equipment pick up with vendors as needed.
• Trends calls to identify patterns and opportunities for improvement.
• Deploys staff to address patient and family needs as required.
