Job summary

GBMC HealthCare is seeking a Hospice Home Care Nurse to provide nursing and supportive care for terminally ill patients and their families in home or facility settings. The role reports to the Team Manager and focuses on comprehensive home-based palliative care planning and execution.

Qualifications

• Diploma, ADN and/or BSN from an accredited institution.
• Current Maryland State license as a Registered Nurse.
• Two years of nursing experience, or equivalent combination of education and experience.

Responsibilities

• Completes initial assessment of patient and family to ascertain home care needs.
• Prepares and implements the care plan in collaboration with the Interdisciplinary Group.
• Provides direct nursing care to patients in order to maintain highest level of care with the primary physician.
• Plans and supervises the delivery of care by the HHA.
• Cooperates with Clinical Manager in utilization of other team members and resources; updates primary physician and interdisciplinary team.
• Uses problem-oriented charting to ensure continuity of care in home, facility, inpatient, or respite settings.
• Teaches family, facility staff, and caregivers nursing skills including pain control, meds, injections, IVs, dressing changes, and vital signs.
• Ensures arrangements for necessary equipment in facility settings are made.
• Attends to patient, family and facility at the time of death.
• Maintains professional relationships with facility staff and participates in care plan meetings.
