Qualifications

• Diploma/ADN/BSN from an accredited institution.
• Current Maryland State license as a Registered Nurse.

Responsibilities

• Triages all patient and caregiver calls, assisting in solutions and education.
• Serves as liaison with physician offices on behalf of the care team.
• Provides immediate response and emotional support to families at time of death call.
• May assist in coordinating changes in patient level of care.
• Communicates with insurance case managers on behalf of the care team; documents discussions in detail.
• Assists with pharmacy issues and orders medication refills; resolves system communications breakdowns.
• Orders supplies and durable medical equipment as appropriate and documents in the EMR.
• Coordinates equipment pick-up with vendor representatives as necessary.
• Trends calls to identify patterns and opportunities for improvement; deploys staff as needed.
