GBMC HealthCare is seeking a Triage RN to triage patient and caregiver calls, provide symptom palliation and education in collaboration with case managers and physicians.

The role requires Maryland RN licensure and 1–2 years of nursing experience, preferably in Home Health Hospice or triage. Strong EMR skills and patient-family-centered approach are essential.

Qualifications

• RN licensure in Maryland required.
• Nursing degree (Diploma, ADN and/or BSN) from an accredited program.

Responsabilités

• Triages patient and caregiver calls, provides education and palliation of symptoms.
• Acts as liaison with physician offices for care team.
• Provides emotional support to families at time of death calls.
