Job summary

Qualifications

• Diploma, ADN and/or BSN from an accredited institution.
• Current Maryland State license as a Registered Nurse.
• One to two years of nursing, Home Health Hospice or triage experience required.

Responsibilities

• Triages all patient and caregiver calls, assisting in solutions and education.
• Acts as liaison with physician offices and care team for coordination.
• Provides emotional support to families and coordinates changes in care level.
• Documents discussions in detail and coordinates with insurance case managers.
